Your First Appointment at Batesville Surgery & CT Imaging

Please arrive at the office 15 minutes early to submit or fill out our patient registration and information forms - unless you print them from our website and complete them in advance. You will need to bring any insurance forms and your insurance card with you. If your insurance company requires a referral, you must bring it with you to the appointment unless your primary care physician has forwarded it to our office.
